Position Overview:
The Pool Nurse – Domiciliary Services will play a key role in the home healthcare team, delivering direct nursing care to patients as per their physician’s care plan. The nurse will support patient care needs, promote recovery, and ensure continuity of care in the home setting, ensuring the highest standards of nursing practice.
Key Responsibilities:
- Patient Care: Provide personalized nursing care at patients’ residences according to the care plans provided by the physician.
- Medical Procedures: Administer medications, dress wounds, monitor vital signs, and perform other nursing procedures as necessary.
- Patient Assessment: Regularly assess patients’ conditions, reporting any changes to the supervising nurse or physician.
- Patient and Family Education: Educate patients and families about medication administration, hygiene, wound care, and other health instructions to promote better recovery.
- Documentation: Maintain accurate and up-to-date patient records, ensuring thorough documentation of care provided.
- Collaborative Care: Work with doctors, therapists, and other healthcare professionals to ensure a coordinated and holistic approach to patient care.
- Flexibility and Availability: Work flexible shifts based on patient demand, availability, and the nature of care required.
- Patient Safety: Follow strict infection control protocols and ensure patient safety standards are met at all times.
- Patient Handling: Assist with patient positioning and transportation as needed.
Knowledge & Competencies:
- Nursing Expertise: Strong understanding of basic and advanced nursing practices.
- Home Healthcare Standards: Familiarity with home healthcare standards and protocols, ensuring the best care in the home setting.
- Infection Control & Patient Safety: Knowledge of infection control, patient safety, and proper medication management.
- Communication Skills: Ability to communicate effectively with patients, families, and the healthcare team, ensuring clarity and empathy in all interactions.
- Clinical Skills: Competence in administering injections, wound care, IVs, catheterization, and managing emergencies.
- Confidentiality & Integrity: Maintain confidentiality, demonstrate punctuality, and uphold a high level of professional integrity.
Requirements:
- Academic Qualification: Diploma or Bachelor’s in Nursing from a recognized institution. A valid PNC (Pakistan Nursing Council) License is required.
- Professional Certification: BLS/First Aid certification is preferred.
- Work Experience: At least 1-2 years of clinical nursing experience, preferably in home care, medical-surgical, or community nursing.
